Your Commitment:
  • Creates enriching an fulfilling experiences for clients and their families in various settings.
  • Reviews and implements care plans approved and provided by the Care Coordinator with precision.
  • Collaborates with Care Coordinator to optimize our clients’ experiences and care.
  • Demonstrates competency within job description boundaries for safe and effective care provision.
  • Utilizes digital tools for seamless communication and legal documentation in client care.
  • Adheres strictly to safety protocols and equipment guidelines
Skills & Experience:
  • Graduate of Recognized Nursing Program: Holds a certification from an accredited Nursing program.
  • Minimum 1 Year of LPN/RN Experience: Possesses a minimum of one year of experience as a Licensed Practical Nurse (LPN) or Registered Nurse (RN), with additional home health care experience considered a valuable asset.
  • 1 Year of Prior Home Care Experience: Assisted clients with daily activities, including meal preparation, personal care, mobility assistance, light housekeeping, companionship, and more, in alignment with individualized Care Plans.
  • Current CPR Certification and First Aid: Possesses up-to-date Cardiopulmonary Resuscitation (CPR) Certificate and First Aid training.
  • Recent Negative TB Test: Completed a TB test within the last two years to ensure a safe healthcare environment.
  • Valid Driver’s License and Access to a Vehicle: Holds a valid driver’s license and owns a car, essential for providing mobile care to clients.
